The Admission Procedure
At the time of admission, the resident and/or responsible party will need to complete an admission agreement. Check with the Admissions Coordinator if you have any questions. The following information about the resident will be needed: * Residents Date of Birth * Physician * Social Security Card, Medicare Card and, if applicable, Medicaid Verification (we will make copies at the facility) * Health Insurance Card with billing address, phone numbers and policy numbers * Names, addresses and phone numbers of those to notify if there is an emergency * Copies of Advance Directives and/or Medical Treatment Plan If a resident is coming to our facility from home, the family will need to coordinate obtaining residents medical history, current physical, and doctor's orders. The Admissions Coordinator will be able to explain and give you the necessary forms.
What To Bring?
We suggest that you leave at home any items that are of value. Because of the many activities in which a resident is involved (such as physical therapy, whirlpool bath, etc.), we suggest you leave rings, watches, etc. at home. Enclosed in the admission packet you will find our policies regarding lost articles. We can professionally mark all clothing with a small iron-on to ensure each piece of clothing can be identified.
Clothing:
*Undergarments for 7 days *Comfortable slacks, shirts, dresses, blouses, and skirts *Sleepwear *Sturdy shoes with low heels, non-slip soles *Slippers *Socks *Sweaters (front opening) *Coat, gloves and hat for outings and appointments *Jogging suit/sweats for physical therapy The facility will provide free laundry service for the resident. If you wish to take it home to wash, please inform the staff and a sign will be posted that the family will do the laundry. You will also need to bring a dirty clothes hamper for the clothing to be stored until family comes in to pick it up.
Hygiene Items:
*Brush/comb. *Dentures and denture cream *Make-up, etc.
Room Decorations:
We realize that our facility will be home for a period of time. We encourage you to personalize your loved one's room with favorite pictures, a plant, radio, favorite chair, clock, afghan, etc. We provide televsion and cable service at NO CHARGE. Many rooms have jacks for phone service and you may call QWEST to activate a phone line. The number is 1-800-224-1111. We do not provide the phone and all billing will be handled through QWEST. *Please keep in mind State Safety Regulations require that residents may not have in their rooms any extension cords, items placed on the over-the-bed light, or any plastic bins for clothing. You may use a wire bin. We appreciate your cooperation in keeping our building free of fire hazards.
Medical Equipment:
Wheelchairs, walkers and canes are available at the facility. However, you are encouraged to bring these items from home if you have them. Please have equipment engraved with the resident's name.
Meals:
The resident is provided three meals a day and three snacks a day. You may dine with your loved one for a minimal charge of $2 per meal. Please check with either the nurse's station or receptionist to order a meal at least two hours before the scheduled mealtime.
Medications:
All non-prescription drugs such as laxatives, topical cream, etc., must be given to the charge nurse at the time of admissions and any other time thereafter. Services at Kolob Care and Rehabilitation of St.George are available to all persons residing in its service area without discrimination toward age, race, color, national origin or religion.
